HC Augusta LLC · Struck by flying object

Employee was climbing a fixed ladder located outside and the wind blew sand behind EE's safety glasses and into EE's eye. A grain of sand became lodged in EE's cornea and had to be removed by a doctor. The doctor prescribed an antibiotic ointment.

EE was climbing a ladder and felt left elbow pop when EE got to the top. EE has been diagnosed with tennis elbow.

Employee attempted to remove sand from a 1" pin hole by sticking a finger in the hole while another employee was sliding the tow bar into the equipment. The employee's pointer finger was subsequently pinched in the hole causing a laceration that required stitches.

A 289D Cat Skid steer belonging to Gerke excavating was working on property owned by Hi Crush Augusta when it suffered a mechanical failure that resulted in said piece of equipment catching on fire. An apparent hydraulic line failure allowed oil to come in contact with the exhaust and/or turbo of the skid steer, causing a fire to start.

Employee was using a cutting torch to cut through 1/4" steel on a UHMW-lined conveyor chute. The UHMW ignited and burned inside the chute, which caused the conveyor belt at the top of the chute to start on fire. Employee was unable to extinguish the fire with a fire extinguisher. The fire department was subsequently called to extinguish the fire.

The employee was standing on the floor and turned EE's body to reach for a pail when EE's knee twisted and popped. EE was not holding anything at the time of this non-contact injury.

Heat exhaustion. Staff member was changing screens on the screen deck. The temp in that area was just over 100 and the staff member was in that area for about 45 minutes.

Ice build-up on a conveyor. a fellow staff member was breaking ice off the conveyor with a hammer and a piece of ice traveled horizontally about 5 feet and struck the staff member in the upper lip. The employee received three stitches and was returned to the job site with no restrictions. this staff member was wearing all PPE at the time of injury.

Employee was connecting conduit in a hole he dug and sand fell in on side and hit him in the hip area and injured his hip. He said the hole was only 3.5' to 4' deep but he was on his knees when it happened.

Employee stated he has Tendonitis of his elbows (R/L). Employee stated it happened while at work using scrapper in a sand hopper. Employee reported this injury on June 17, 2014 with a doctors slip. it is unknown when is return will be.

EE was climbing down a ladder and after he reached the ground he turned and slipped on some snow that he just brushed off a railcar. This incident happened in our 500A silo. EE fell landing on his tailbone. He was taken to the local hosp. and was released the same day. At this point he is on light duty for two weeks as per doctors orders

Equipment 170A was locked out and staff was wearing the proper PPE. Employee was replacing screens with the assistance of another EE. Employee had his finger on the door cover as other EE shut the door. I took employee to the local hospital where he received five stitches. The doctor placed no restrictions on employee.

On 12/5/2013 ee stated in writing that he strained his back pulling up on a spud bar. He did not want to see a doctor and finished his shift. On 1/16/2014 he seen a Doctor for a back pain. He was off by the Doctors orders for 5 days and came back to work on 1/21/2014 with restriction. He will be reevaluated by the Doctor on 1/24/2014
