Over-exertion in lifting objects

EE was picking up a stone approximately 150 lbs., twisted to put it on a pallet, and felt lower back pain.

Struck by rolling or sliding object

EE picked up stone approximately 80-120 lbs., set it on EE's thighs, rolled stone towards chest, adjusted grip on the stone, and set it on the pallet. When the stone rolled up and towards EE's chest it contacted EE's left ribs, resulting in a fracture.

Over-exertion in lifting objects

Employee was stacking large stone when EE felt pain in EE's elbows and wrists. The condition worsened over time. Employee sought medical care on 10/29/21. EE was diagnosed with joint pain. Injury was determined reportable on 10/29/21.

Struck by falling object

On Wednesday, 5/12/21, at 2:48 pm, employee moved a stone forward on the feeder conveyor towards the breaker. They lifted it with the lifter table. EE then attempted to spin the stone with left hand when the stone slipped off the lifter table and fell. When the stone fell it pinched their left 2nd digit in between it and the feeder conveyor resulting in a laceration/open wound.

EE was pulling stone on a breaker feeder table when a small piece broke off, fell off the table, and contacted the top of EE's left ankle resulting in a laceration.

At 9:45 am on Friday, 4/16/21, employee was attempting to climb over a conveyor, when a foot became stuck in a pallet, employee lost balance, and fell sideways onto the concrete floor, hitting left shoulder on the floor.

EE reported left shoulder pain to the supervisor. EE felt pain in the shoulder when EE was swinging a hammer to hit a chisel on a piece of stone.

Employee was stacking stone on a pallet when a piece of stone fell off the pallet and struck EE in the hip and foot resulting in contusions.

Employee was sorting and stacking stone in the quarry. EE lifted a 50 lb. stone and felt pain in EE's lower back.

At 4:00 pm on Tuesday, 6/16/20, EE was feeding a splitter at the Hamilton Shop. EE was turning a stone on the turn table on the feeder table when the stone moved suddenly and EE's left 4th finger was pinched between the stone and a piece of steel on the feeder table, resulting in a laceration.
