EQUIPMENT HAD FROST/ICE ON IT. WORKER FELL FROM CRUSHER TO THE GROUND. WORKER FELL ON FACE AND CHEST & WAS UNCONSCIOUS WITH MULTIPLE INJURIES. WORKER WAS TAKEN TO HOSPITAL BY AMBULANCE.
