Fall to the walkway or working surface
Slip and fall was at the base of a small pile of dirt and rock. When turning to face a new direction a loose rock moved and EE fell injuring EE's elbow.

SLIPPED ON SMALL HILL, FEET SLIPPED, EE FELL ON BUTTOCKS. NECK PAIN THAT EVENING WHICH PERSISTED.

EE NOT FOLLOWING SAFTY INFECTIONS CAUGHT HIS THUMB BETWEEN DRIVE BLOCK AND DRIVE HEAD OF CHURN DRILL WHEN HE WAS DISTRACTED.
