At approximately 09:55 am an employee was in the Crystal 290 heading preparing to survey. The employee was walking towards the face when loose material fell off the right rib towards the back striking the employee.

Miner stood up after loading lifter holes of face round felt soreness in his back.
Miner was assisting at the face opening anfo bags for loading a round, a pressurized anfo pot was being used. The pot ran out of anfo the miner loading the round lost his grip on the hose; the hose then whipped back and struck the miner opening the anfo bags in the face lacerating his upper lip.
Miner using jackleg, strain back. Did not report accident to supervisor till off site in Colorado. Saw doctor in home state on May 13th and was reported as workers comp claim.
Employee was walking down to mill - he slipped and fell down twisting his ankle, was taken to Anchorage Alaska - determined that the ankle was fractured.

Employee climbed up on our xanthate tank without the use of a ladder; when he descended his elbow caught the edge of the tank and strained his left shoulder.

Employee was sprayed in both eyes when working around sample box. Employee immediately went to eye wash station and flushed eyes out, then supervisor took and flushed eyes again. Left eye was okay, but right eye had possible abrasions. Flew employee out of camp to Anchorage, AK to be seen in ER. Was flown out Sunday, 08/19/2012.
Unknown, employee noticed material in right eye about noon; he went immediately to on-site EMT who flushed out the eye. After 24-hours and no improvement, the employee was flown from camp to Anchorage ER for further treatment.
rehabing timber sets on main haulage of crystal mine 250 level. during shift the ee got something in his left eye.
Dermatitis reaction to unknown material, EE is on two week R&R in Montana. Doctors note of June 21, 2012 states allergic reaction - chemicals - no work for next 2-4 weeks. Received Doctors note June 25, 2012
Mill Operator placed his hand in a pinch point and his left hand got caught in between two pieces of metal
ee was welding and foreign material fell off of his welding mask and entered his eye.
Miner using jackleg to put roof bolts in the back, rock came out of back slid down jackleg hitting ""L"" little finger and left forearm.
Miner was bolting and the leg extended rapidly. The miner tried to catch the machine and got his hand in between the wall and the handle of the machine.
EE was kneeling at face prepping to load face, other miner drove into heading with 4-yard mucker and couldn't see EE, miner stopped mucker and rolled bucket forward onto EE.
this accident was immediately reported by phone. miner went into his work area and noticed the rib had sloughed and immediately came out of the mine and reported it to the mine superintendent.
employee picked up a bundle of split set plates and turned at the waist to set them down straining his back in the process
EE was walking down ramp at 220, he slipped and tried to catch himself and twisted his left knee. February 15th, clinic has requested an MRI, with tentative surgery to knee.
Employee complained of pain to both wrist. Employee left camp for two week rotation and was scheduled to be seen by Hand Specialist in Boise, Idaho, on January 30th. Diagnosis Carpal Tunnel Syndrome - both wrists.
Employee was stepping back away from work table and tripped over electrical cords of the tools he was using; falling over backwards into a drain flange on Tank #4. Employee did not seek medical help till January 14, 2012

Setting up to use jackleg drill; while holding drill with left hand miner had right hand on leg of drill, head came down on right hand striking top of right hand and two middle fingers. Miner flew out of camp on December 19th, was seen and sent for evaluation - x-rays taken on Thursday December 22nd indicated fractures to wrist and finger.
Miner was cutting a hose with his pocket knife when the knife slipped and lacerated his left palm
Employee got out of pickup, slipped on ice and fell backwards falling on both elbows and hitting back of head, injuring ""L"" shoulder
Employee was mucking out the 270 stope access, scaling area again before finishing mucking to bolt. While scaling there was a rock hit him in the head. Going in to see a physician on the recommendation of site EMT.
Employee asked to see EMT about pain in right hand and elbow. During the patient interview process with EMT, employee stated that there had not been any on site injury. Employee has had two previous neck surgery within the last nine years.

The driller, was emptying a 5 foot steel core tube. Some core was stuck in the tube and the EE went to drop the tube down onto a wood block, but had his finger at the bottom of the tube. As a result, the finger was pinched and lacerated.

While using a table saw to cut 4"" x 4"" blocks of wood, the wood kicked back. The block hit his left thumb, pushing back. The thumb was broken, requiring surgury to repair. Security(contractor) was assigned the duties of safety and reporting. A safety audit identified this accident as not being filed at the proper time.
Cutting floor tile with razor box cutter, he slipped and stabbed left palm. This injury was not reported at the proper time. Security (contractor) was responsible for safety and reporting. An safety audit identified that this was not filed on a 7000-1. The injury required sutures, no lost time or days.
The miner states while pulling back on jackleg machine his hand slipped off the handle and he stuck out his arm to break his fall and in doing so he dislocated his right shoulder.
Miner was getting down from haul truck, stepped on a rock and twisted his ankle.
Employee was welding and grinding throughout the day. A particle lodged in his left eye and was irritated.

Employee was walking down a stairway in the Nixon Fork Mine's Process Mill facility. His heel slipped our from under him and he attempted to catch himself. His left hand ring finger became caught in a higher stair grating and the weight of the fall broke and dislocated the finger.

Employee states while drilling a 53 degree up hole the over shot released the tube and the tube struck employee in the chest
