finger smashed in belt

EE was using come along to lift a metal plate to complete repairs on crusher hopper. Chain slipped, and hit EE's knee. Emergency room assessment of injury is bruising to left knee.

EE slipped on ladder in crusher house, while holding chain for hatch door. Pulled chain when slipping and chain smashed right pointer and middle finger. Resulted in broken finger tips.

While moving (tearing down & setting up) crushing plant, using blocks to lift crusher, it slipped catching his right hand, injuring thumb and parts of pointer finger.

After starting the impactor, and while greasing the main impactor bearing, the blow bars just exploded.

WHILE CLIMBING UP THE STAIRWAY LADDER ONTO THE LOADER, MISCUED A STEP AND FELL FORWARD ONTO THE FLOOR HITTING THE CHEST AND RIGHT CHEEK.
