EMPLOYEE REPORTED THAT WHEN CLEANING A PIECE OF REBAR FROM A DISCHARGE CHUTE, A PIECE OF RECYCLED CONCRETE DISLODGED AND SLID DOWN THE CHUTE HITTING EMPLOYEE'S HAND AND FINGER.
EMPLOYEE WAS RUNNING ACROSS QUARRY FLOOR TO STOPA PLUGGED MACHINE AND STEPPED ON A TENNIS BALL SIZED ROCK, INJURING HIS LEFT FOOT. WHEN THE INJURY BECAME MORE PAINFUL, MEDICAL ATTENTION WAS SO UGHT.
