American Asphalt & Grading Co · Struck by... (Not Elsewhere Classified)

Employee was assisting in changing screens on the crusher. When he attempted to pull a screen from a pile, the stack of screens shifted causing one of them to strike the employee's right calf. Injury consisted of dime-size abrasion.

Employee was assisting in tear-down of crusher. In the process of removing a guard, the guard fell and cut the employee's left forearm requiring 6 stitches.

Fall from WA500 wheel loader, coming down from loader slipped and fell, injury to left hand. Raining at the time and area was very wet and muddy from 3 days of rain.

American Asphalt & Grading Co · Over-exertion in pulling or pushing objects

EE was pulling rocks out of crusher feeder belt when his back started to stiffen & he felt pain in lower back, hips & right leg. Also while he was shoveling rock off feeder belt his neck began to stiffen. EE failed to notify Human Resources that he was in the crushing div. when he reported his injury. HR failed to provide required info to crushing div. unitl it was requested.
