Was standing on a step ladder refueling a Dozer. With both feet on the last rung of the ladder, EE went to stepping off with left leg. EE's boot got stuck on the ladder and EE lost balance causing EE to fall. EE fell backwards and struck an oil container then landed on left side on the floor.

EE REPORTED THE NEXT DAY SHOULDER WAS HURTING AND WILL NOT BE COMING TO WORK. EE STATES RIGHT SHOULDER BEGAN HURTING PROGRESSIVELY MORE TO THEEND OF SHIFT. HE DOES NOT KNOW WHAT CAUSED IT.

ON MONDAY, WE RECEIVE NOTIFICATION FROM EE'S HOUSEHOLD THAT EE WOULD NOT BE IN TO WORK BECAUSE HE HAD INJURED HIS BACK. LATER IN A T/C HE SAID HE HAD HURT HIS BACK THE PRIOR TUESDAY MOVING CUT TING EDGES WITH A LOADER. HOSPITAL T/C TODAY SAID DIAGNOSIS IS BACK SPRAIN, AND THEY ANTICIPATE HIS RETURN TO DUTY IN A WEEK.

EMPLOYEE SUFFERED BRUISES, AND CUT HIS HEAD REQUIRING STITCHES. THISHAPPENED WHEN HIS HAMMER SLIPPED WHILE HE WAS GETTING SOME METALLIC OBSTRUCITON OUT OF A CRUSHER WHILE THE PLANT WAS SHUT D OWN, HE LOST HIS BALANCE AND FELL OFF THE CRUSHER PLATFORM.

EE SLIPPED OFF BADGER NO 88 AND KEEP WORKING MENTIONED HE FELL OFF LOADER NEXT WEEK. WENT TO DR IN MARCH BUT DID NOT TAKE ANY TIME FROM WORK. FINALLY ON JULY 1999 HE CEASED WORK BECAUSE OF COM PLAINTS HE RELATED TO THE NOV 1988 FALL. HIS LAST DAY OF WORK WAS JULY 12, 1999.
