ICY STEP ON D8 DOZER CAUSED EE TO SLIP OFF. HIS ARM WAS STUCK AND CAUSED HIM TO HANG BY HIS ARM ONLY. SERIOUS SHOULDER INJURY WAS SUSTAINED. HE WAS CHANGING THE OIL AT THE TIME OF THE ACCID ENT.

TROUBLE SHOOTING THE CONVEYOR BELT, TRYING TO DETERMINE WHERE THE LOSS OF VOLTAGE OCCURRING, CHECKED VOLTAGE ON LINE STARTER,IT HAD ONE DEAD LEG WENT TO CHECK VOLTAGE ON MAIN BREAKERS AND EVERYTHING BLEW UP.
