THE INJURED PERSON WAS BREAKING OUT 4 1/2" DRILL ROD TO COME OFF BOTTOM IN 18 FT. HOLE, 6 1/4" DIA. HE HAD INSTALLED AN ADDITIONAL WATER DEFLECTOR IN THE FORM OF A PIECE OF CONVEYOR BELTING A ND WAS HOLDING IT IN PLACE WITH HIS FOOT WHEN THE BREAKOUT WRENCH ROTATED, CRUSHING HIS FOOT AGAINST THE MAST.
