EMPLOYEE WAS FEEDING MUCK INTO WEIGH BIN MUCK WAS WET AND WOULD NOT FLOW THROUGH HE WAS USING A BAR TO HELP FEED ROCK THROUGH BIN. HE STEPPED ONTO ORE TO GET BETTER LEVERAGE ORE GAVE WAS UNDE GAVE WAY UNDER HIM HIS FOOT WENT THROUGH ORE AND HIT CONVEYOR BELT.

EE HAD LOAD OF MUCK ON DUMP TRUCK. MUCK WAS WET SO EE WAS CLEANING MUCK OUT OF BED OF TRUCK. HIS FOOT SLIPPED & HE FELL. HE TWISTED TO TRY & STOP HIS FALL, PULLED LEG MUSCLE, FELL ACROSS SIDE OF TRUCK & BROKE LEFT ARM.
