EMPLOYEE WAS DUMPING A SEMI LOADED WITH COAL, THE COAL FROZE TO THE BED OF TRAILER, IT BROKE LOOSE ON ONE SIDE CAUSING THE TRUCK TRAILER TO OVER TURN. THE DRIVER RECEIVED LACERATION TO THE HEA D, HE WAS TOOK TO DARVIESS COUNTY HOSPITAL. THE ACCIDENT WAS AT THE SUGAR TREK MINE SOUTH OF CANNELBURG.

EMPLOYEE WAS CLIMBING UP SIDE OF BED OF TRUCK TO CHECK MUD & ROCK STUCK IN BED WHEN FOOT SLIPPED & HE STARTED TO FALL. HE CAUGHT HIMSELF BUT HIT RIGHT LEG ON SIDE OF B ED THAT CREATED SMALL PUNCTURE IN LEG THE GOT INFECTED 2 DAYS LATER. WENT TO HOSPITAL TO GET INJECTION.
