Helper disconnect the water hose at the centrifuge by releasing the camlock fitting. Helper released the camlock, the fitting separated unexpectedly and struck the injured person on the chin, causing a minor laceration.

On the morning of July 21, 2026 at 11:01 the IP was found unresponsive on the deck of DDS-09. Emergency response team was called, IP was transported to hospital for evaluation
IE was transporting a concrete block with a forklift and chain when the chain slipped, causing the suspended block to fall onto the employee's right foot.
