The employee was tasked with tapping the side of the hopper below the heat exchanger. After tapping on the hopper the employee opened the hatch the #2 rotary air valve and reached upward toward the valve where employee's right thumb inadvertently made contact with the vanes removing the tip of employee's thumb.

Employee was checking the fuel on a welding machine, which is located on the back of a flatbed pick-up. Employee climbed up the driver's side of the bed, instead of the rear of the bed. Employee slipped on the pick-up's fuel neck, which is located on the bed. Employee tripped and fell to the ground, landing on his right foot. His ankle broke/dislocated upon impact.

EE was trying to move steel plate back into place when he felt a pull in lower abdomen area. Went to DR and diagnosed with a strained muscle. After further testing it was found there was a hernia in the abdomen area and that surgery would be needed. After diagnose was made surgery was put off until 4/20/12 by Doctor.

Employee was using a grinder with a cutting disc to cut a piece of angle iron. As the Employee was finishing the cut, the disc caught part of the angle iron causing the disc to put across his left arm. An inspection of the grinder showed the ginder was guarded and all safety equipment ppe were being used.

EE was cleaning grizzly bars off a hopper. The hopper is fed by a feeded belt. When reaching for a rock the EE's hand was pinched between the rock and the headpulley of the feeder causing lacerations to EE's right hand.

THE EE WAS CLEANING MUD OUT OF A SEALING AUGER WHEN HIS RIGHT INDEX FINGER CAME IN CONTACT WITH ONE OF THE BLADES ON THE AUGER, CAUSING A CUT JUST BELOW THE SECOND JOINT. THE AUGER WAS LOCKED OUT. THE CUT REQUIRED 5 STITCHES. THE EE WAS NOTWEARING HIS GLOVES AT THE TIME OF THE INCIDENT.
