Using a shovel to clear blockage on belt. it slipped and caused laceration on left index- 1 stitch and glue

Employee suffered lacerations to right hand while training a conveyor belt.

Employee was exiting light duty pickup. Shut right middle finger in door. Stitches required from impact laceration.

Employee was working on a conveyor roller, stepped back into hole and strained left knee.

Employee was entering a John Deere skid steer when the EE's left foot became caught between the bucket and the frame causing the employee to fall, twisting left knee.

Employee was sitting in articulating haul truck and claims received cervical strain when being loaded.

The employee was tasked with tapping the side of the hopper below the heat exchanger. After tapping on the hopper the employee opened the hatch the #2 rotary air valve and reached upward toward the valve where employee's right thumb inadvertently made contact with the vanes removing the tip of employee's thumb.

Employee was walking down the drivers side walkway of an excavator, missed step with right foot falling to the ground. Fall caused broken ankle, femur and injured hip to right side of body. injured employee failed to use OEM handrails(3-points of contact) when lost balance.

Repetitive Motion- removing old struck bags from dust collection baghouse, using repetitive motion for a approx. 4-6 hrs. Injury actually occurred on the 15th but was not reported until the 16th (next day).

The employee was dumping a bucket of sampled material into the sample splitter when they felt a pain in their back area.

Employee was cleaning underneath # 19 conveyor belt and catwalk. When employee raised EE's head, EE struck the catwalk structure with the head. Employee felt pain to the neck.

Employee was climbing down a ladder located behind the #10 Belt. There was a two foot gap between the ground and the bottom rung of the ladder. Employee slipped off the bottom rung and hit lower back on a concrete wall located next to the ladder.

Employee strained left knee while turning off water valve in lime room.

Employee was changing teeth on an excavator bucket. Employee struck employee's hand with a hammer.

Employee was taking a sample from the kiln. Employee noticed a pain in right shoulder. Doctor placed employee on restrictive duty.

Employee was removing a drive chain from a gearbox with a pry bar. The pry bar slipped, smashing left index finger between the pry bar and gearbox. Employee's left index finger was fractured.

Employee was cleaning snow off of #2 belt. As EE was walking back to the truck EE slipped on a snow packed surface. Employee broke a bone in left ankle as EE fell.

Employee was traveling up the stairs of #10 belt. While approaching the platform EE slipped and fell forward. EE cut EE's head on the metal head box of #10 belt.

Employee was removing the motor connection cover. He had a box knife on top of the connection cover. When he broke loose the cover he cut his left hand with the box knife.

Employee was lifting a 5 gallon sample bucket of light weight aggregate. She felt a strain in her back. Doctor put her on restricted duty.

Employee was checking the fuel on a welding machine, which is located on the back of a flatbed pick-up. Employee climbed up the driver's side of the bed, instead of the rear of the bed. Employee slipped on the pick-up's fuel neck, which is located on the bed. Employee tripped and fell to the ground, landing on his right foot. His ankle broke/dislocated upon impact.

While trouble shooting a possible short in a wire that was located in a cable tray EE inadvertly caused the wire which had became bare to contact the cable tray and caused a shock.

Years of repetitive motion caused Paresthesia Bilateral Carpal Tunnel Syndrome. Surgery is needed.

EE was trying to move steel plate back into place when he felt a pull in lower abdomen area. Went to DR and diagnosed with a strained muscle. After further testing it was found there was a hernia in the abdomen area and that surgery would be needed. After diagnose was made surgery was put off until 4/20/12 by Doctor.

EE was stepping up into basket of manlift when felt a pop in left knee. EE sought medical attention on 10-15-11, diagnosis - knee sprain. Pain contiuned EE returned to Doctor and a diagnosis of Torn Meniscus and that surgery was need on 11-15-11.

Employee failed to follow known safety policies and procedures while attempting to fuel a Hitachi 750 Excavator. Impairment is 2 broken elbows, full extent unknown at this time.

Employee was watering down kiln dust. Employee decided to check the dust out closer and walked into some of the hot dust.

Employee was using a grinder with a cutting disc to cut a piece of angle iron. As the Employee was finishing the cut, the disc caught part of the angle iron causing the disc to put across his left arm. An inspection of the grinder showed the ginder was guarded and all safety equipment ppe were being used.

EE was squatting to open an inspection plate to the coalmill, it would not open, therefore he started to tug harder and felt a pull in his lower back.

EE was closing doors on top of railcars, when his left foot slipped off the top of the door and twisted his ankle.

EE was cleaning grizzly bars off a hopper. The hopper is fed by a feeded belt. When reaching for a rock the EE's hand was pinched between the rock and the headpulley of the feeder causing lacerations to EE's right hand.

EMPLOYEE WAS WORKING ON ARM OF LOADER. A PIN CAME OUT OF THE LOADER & THE ARM DROPPED. THE LOADER ARM TRAPPED THE EE'S HAND BETWEEN THE ARM & TIRE CAUSING A LACERATION ON HIS RT. THUMB.

EMPLOYEE WAS HANDLING A PIECE OF METAL WHEN IT SLIPPED & CUT HIS FINGER. THE CUT REQUIRED 6 STITCHES IN HIS RT. PINKY FINGER.

THE EE WAS CLEANING MUD OUT OF A SEALING AUGER WHEN HIS RIGHT INDEX FINGER CAME IN CONTACT WITH ONE OF THE BLADES ON THE AUGER, CAUSING A CUT JUST BELOW THE SECOND JOINT. THE AUGER WAS LOCKED OUT. THE CUT REQUIRED 5 STITCHES. THE EE WAS NOTWEARING HIS GLOVES AT THE TIME OF THE INCIDENT.

EMPLOYEE WAS ATTEMPTING TO REMOVE A PIN OUT OF A SCREEN WHEN HE HIT HIS THUMB WITH A HAMMER.

EMPLOYEE IN VEHICLE #1 GOING SOUTH BOUND OUT OF DRIVE WAY. COLLIDED WITH CONTRACT CARRIER ENTERING DRIVE GOING NORTH IN LEFT HAND CURVE.

EMPLOYEE WAS CLEANING CRUBBER TANK WITH HIGH PRESSURE CLEANER. EMPLOYEE PUT RIGHT HAND OVER THE CLEANING WAND AND PULLED THE TRIGGER, THERE WAS A PIN HOLE IN THE WAND WHICH CAUSED AN INJECTIO N TO THE HAND.

DRIVER DID NOT LOWER TRUCK BED DOWN COMPLETELY AND WHEN GOING UNDER THE BRIDGE THE BOX HIT THE BRIDGE AND STOPPED THE TRUCK. THE CAB CAME UP AND HIT THE BRIDGE AND THEN SLAMMED DOWN CAUSING T HE DRIVER TO HIT HIS MOUTH ON THE STEERING WHEEL.

EMPLOYEE WAS REMOVING OLD REFRACTORY FROM THE LIKN WHEN A PICE OF MATERIAL ENTERED EMPLOYEES RIGHT EYE.EMPLOYEE WAS WEARING SAFETY GLASSES WITH SIDE SHIELDS.EMPLOYEE SHOULD HAVE BEEN WEARING G OGGLES WHICH WERE PROVIDED.
