EE WAS OPERATING A FARM TRACTOR. THE AIR CONDITIONER WAS RUNNING LOW ON FREON. BEFORE THE A/C COULD BE RECHARGED THE A/C STOPPED COOLING. EE'S AT THE SITE PROVIDED FIRST AID AND THE EE WAS TRA NSPORTED TO THE LOCAL HOSPITAL EMERGENCY ROOM. THE EE WAS TREATED AND RELEASED THAT SAME DAY . THE EE RETURNED TO WORK.
EE WAS CUTTING THE TOP OFF OF A FIVE GALLON CONTAINED WHEN THE KNIFE CAME IN CONTACT WITH THE EE'S LEFT LEG. THE EE WAS TREATED AT THE LOCAL CLINIC AND RETURNED TO WORK.

EMPLOYEE WAS CLEANING COVER ON A PUMP BARGE WHICH WAS PULLED TO BANK. HE REPLACED THE HOLE COVERAND STEPPED ON IT. THE HOLE COVER GAVE WAY, SCRATCHING HIS LEFT LEG. THE EMPLOYEE RECEIVED MEDIC AL TREATMENT AT THE LOCAL MEDICAL CLININC AND RETURNED TO WORK.
EE CLIMBED ON A D-9 DOZER TO CHECK FLUID LEVELS. HE REACHED FORWARD AND PULLED HIMSELF UP WHEN HE FELT A PAIN IN HIS BACK. LATER THAT MORNING AS THE PAIN PROGRESSED, HE REPORTED TO THE DISPENS ARY. HE CONTINUED UNDER DOCTOR'S CARE WHILE AWAY FROM WORK. HE WAS RELEASED AND REPORTED TO WORK ON 5/12/99.

EE REPORTED CHEST PAINS AT HOME ON FRIDAY EVENING. REPORTED FOR WORK ON SAT MORNING TO WORK AS TEMPORARY SUPERVISOR. AS HE WAS MAKING JOB ASSIGNMENT HIS CHEST PAINS INCREASED AND HE WAS TRANSP ORTED TO DR OFFICE AND THEN TO BAPTIST HOPSITAL IN LITTLE ROCK. EE CONTINUES TO THE UNDER DR CAR AFTER SUGERY.
EE WAS CLOSING DOOR ON NEWLY DELIVERED BULLDOZER WHEN THE DOOR KNOV (LEVER) BROKE OFF. THIS LEFT A THREADED SHAFT EXPOSED WHICH LACERATED HIS RIGHT HAND. THE EE WA SENT TO CLINIC WHERE HE RECE IVED SUTURES AND RETURNED TO WORK.
EMPLOYEE WAS ASSISTING WITH THE UNLOADING OF DRUMS FROM AN EXTENDABLE BOOM TRUCK TO THE GROUND. THE TRUCK BEGAN TO TIP OVER AND THE EMPLOYEE JUMPED CLEAR OF THE TRUCK. EMPLOYEE RECEIVED MEDICA L TREATMENT AND RETURNED TO WORK. ON JUNE 3, HE WAS PLACED ON PRESCIPTION MEDICATION.
EE WAS CHECKING THE OIL ON HEAVY EQUIPMENT IN THE MINES. HE WAS ON A SAND HILLAND WAS SWEATING SO HE TOOK OFF HIS GLASSES TO WIPE OFF HIS FACE. AND THINKS HE HAD SAND ON HIS HANDS WHICH GOT IN TO HIS EYES. HE REPORTED TO THE PLANT DISPENSARYWHERE THE EYE WAS TREATED WITH PRESCRIPTION MEDICATION.
EE WAS DRIVING IN THE MINES & WENT TO ADJUST THESIDE MIRROR AFTER IT HAD BEEN STRUCK BY A TREE LIMB. THE MIRROR WAS BROKEN & HE CUT HIS FINGER ON THE OF THE SHARDS. HE REPORTED TO THE PLANT DISPENSARY WHERE THE LACERATION REQ SUTURES
