Hooking chain on turntable of saw, operator told loader operator to lower the boom on loader. The chain pinched end of finger with chain. Replaced short chain with long chain.

988F LOADER WAS BEING USED TO PULL LEDGE DOWN. WHILE PULLING EMPLOYEE WAS PLACING ROCKS IN CRACK. CRACK CLOSED QUICKLY PINCHING RIGHT MIDDLE FINGER RESULTING IN LACERATION.

SAW PUMP NOT PUMPING, SUCTION HOSE WAS VIBRATING. SAW WATER SPITING OUT AT CAP TOP END. TURNED OFF MOTOR THEN STARTED TO MOVE PUMP OUT OF THE WAY. CAP POPPED OFF, WATER AND MUD EVERYWHERE. I T URNED AWAY FROM PUMP AND RUSHED TO OTHER WATER, TOOK SHIRT OFF AND WASHED IT OFF MYSELF.

EQUIPMENT CORE DRILL PERSONS WORKING ON DRILL. WHILE DISCONNECTING DRILL RODS WAS STRUCK BY A CRESENT WRENCH THAT WAS LEFT SITTING IN A BAD POSITION ON MACHINE. EE PULLED DOWN PN WRENCH TO UNSCREW NEW ROD. WRENCH HE PULLED DOWN ON HITTHE CRESENT WRENCH THAT WAS SITTING ON MACHINE CAUSING WRENCH TO FLY UP AND STRIKE EE ON THE RT. SIDE OF HEAD, IN EYE AREA.
