AFTER FINISHING CLIPPING A CONVEYER BELT, THE EMPLOYEE STEPPED BACKWARDS AWAY FROM THE CONVEYER. THE EMPLOYEE TWISTED WRONG WHILE WALKING BACK TO HIS LOADER. HE DID NOT FALL OR STUBLE AND THER E WAS NO OTHER APPARENT INJURY.
INJURED WAS LEAVING SHOVEL AFTER LOADING TRUCK. THE ROAD OUT OF PIT IS STEEP. INJURED DOWNSHIFTED AND TRUCK LURCHED. THIS CAUSED LARGE CHUNK OF MATERIAL TO SHIFT BACKWARDS, WHICH CAUSED MORE L URCHING. TRUCK THEN STARTED ROLLING BACKWARDS TOWARDS WATER IN THE BOTTOM OF PIT. INJURED THOUGHT TRUCK WOULD GO INTO WATER. HE STEERED TRUCK INTO BERM. TRUCK ROLLED ONTO SIDE CAUSING INJURIE

EMPLOYEE WAS WORKIGN AROUND SCREENING PLANT WHEN HE TRIPPED AND SLIPPED UNDER GUARDING AT GROUND LEVEL WHEREBY BECOMING ENTANGLED WITH CONVEYOR.
EMPLOYEE WAS OPERATING A 35 TON HAUL TRUCK. EMPLOYEE WAS TRYING TO CUT A HAUL ROAD SHORT WHEN THE TRUCK BECAME UNSTABLE CAUSING THE FRONT WHEELS TO COME OFF THE GROUND AND HE TRIED TO GRAB SOM ETHING IN THE CAB TO BRACE HIMSELF.
