USING IMPACT WRENCH TO REMOVE BOLTS FROM SCRAPERTRACTOR BELLY PAN WHEN FINAL BOLT CAME QUICKER THAN EXPECTED CAUSING PAN TO FALL AND CATCH LEFT HAND BETWEEN GROUND, PAN AND IMPACT WRENCH.

WHILE DRIVINGA TEREX SCRAPER, HYDRAULIC FLUID LINE RUPTURED, CAUSING EQUIPMENT TO CATCH FIRE. EMPLOYEE STOPPED EQUIPMENT AND JUMPED FROM CAB (WHICH IS APPROX 8' FROM THE GROUN). THE CAB WAS, BY THIS TIME, ENGULFED IN FLAMES.
