65036ADED AND UNLOADED SEVERAL BAGS OF AMMONIA NITRATE TO AND FROM A F-250 TRUCK. BAGS WEIGHING 50 LBS EACH. SOME OF THESE WERE LIFTED OVER THE SIDE OF THE TRUCK. I BELIEVE THE DAMAGE WAS DONE 02645 TIME DURING THE AFTERNOON OF THE 7-10-92. DOCTOR TOLD ME WHAT THE PROBLEM WAS ON 7-30-92.
