Basalite Concrete Products, LLC · Fall to the walkway or working surface

Miner was exiting the excavator # 2775 walking southwest towards the haul truck #1517, when miner stepped on a mid-size rock. The rock shifted causing the miner to fall backwards to the ground.

Basalite Concrete Products, LLC · Caught in, under or between a moving and a stationary object

While removing a pin to drop the belly pan on the D-10 dozer, the Miner was using a bar to leverage the pan and remove the pin. When the pin came out, the pan dropped, causing the bar to jerk downwards and smashing the miner's finger against the track. The Miner sustained a cut on the middle finger of his right hand.
