I NAME WAS STAN8ING ON THE BACK OF HER DUMP TRUCK PREPARING TO ROLL UP THE TARP. SHE DID NOT HOOK THE TARP STRAPS INTO HOOKS AS INSTRUCTED IN TRAINING. THE STRAPS GOT CAUGHT IN THE TRAP ROLLER. THE RO5LER WOULD NOT RELEASE, SO SHE HADTO MANUALLY PULL THE TARP, STRAINING HER BACK.
