EMPLOYEE WAS IN THE PROCESS OF WELDING SOME METAL TO AN EXCAVATOR BUCKET. HE PICKED UP A PIECE OF HEAVY METAL AND ALLEDGES HE FELT PAIN IN HIS STOMACH. IT WAS FIRST THOUGHT THAT HE HAD A HO LE IN HIS STOMACH OR A GALL BLADDER PROBLEM, BUT AFTER SEVERAL DAYS OF TESTING THE DR. SAYS THAT THE MUSCLES AREA PULLED APART IN STOMACH AREA. THAT IS WHY THE DELAY IN COMPLEATING THIS FORM.

THE EMPLOYEE WA8 CLIMBING DOWN FROM AN END LOADER LEAVING HIS SHIFT. AS HE CLIMBED DOWN HE STATED HE FELT SOMETHING PULL IN UPPER INNER THIGH ON LEFT LEG. THE NEXT MORNING HE COULD NOT GET OUT OF BED. CALLIN1 AND REPORTING THIS TO HIS FOREMAN.
