EMPLOYEE WAS STANDING ON TOOL BOX MOUNTED ON BED OF 1/2 TON PICKUP TRUCK. P/U WAS POSITIONED NEAR BOOM OF BACK HOE SO EMPLOYEE COULD REACH HEIGHT OF CRACK ON BACKHOE BOOM TO WELD SAME. WHILE W ELDING, EMPLOYEE STEPPED TOO CLOSE TO EDGE OF TOOL BOX CAUSING EMPLOYEE TO LOSE BALANCE AND FALL TO GROUND, STRAINING TRACK OF BACKHOE WITH LEFT SIDE OF RIBS.
