Employee was operating a wheel loader when the bucket of wheel loader got caught on the grade causing the employee to hit head on the windshield causing lacerations to face.
Employee was getting ready to refuel an Excavator. Employee placed the hose and pump handle from the fuel truck on the Excavator platform and was about to climb the ladder to fuel when the hose and pump handle fell from the Excavator striking the employee in the left cheek. The handle caused a small cut and contusion to the left cheek.

employee was loading a steel drill bit into the back of a truck. They propped the drill bit on the truck and placed hand down on the bed of the truck, while doing so the bit slipped off of where it was propped and landed on the employee's right index finger causing a contusion. The accident was reported and employee was sent to urgent care clinic to be seen by the doctor.
Employee was walking down a slope in the road to crusher and felt a pop in right leg causing employee to fall to the ground landing on side. Employee worked entire shift and did not report the accident to supervisor. Employee went to the ER later that night. Employee called Supervisor the next morning to report employee had broken leg.
