WHILE ON HIS WON TIME OUR EMPLOYE TOOK IT UPON HIM SELF TO CHECK THE HEAVY EQUIPMENT, HE DID THIS BY MEANS OF HIS PERSONAL THREE WHEELER HIS FOOT STRUCK A ROCK AND HIS SMALL TOE WAS REMOVED. H E WAS NOT WEARING PROPER SHOES AND WAS AT THE MINE SITE WITHOUT OUR KNOWLEDGE.
