A 3/8" HELPER CHAIN CONNECTED TO A 1 1/2 TON RATED COME-ALONG HAD BEEN ATTACHED TO THE POINT AUGER OF THE EXTENDER IN AN ATTEMPT TO REMOVE IT. THE 3/8" HELPER CHAIN BROKE AND STRUCK THE WORKER IN THE LEFT ELBOW.
