EE was trying to dislodge mantrip that was stuck in mud, when he fell injuring his back and left leg. He then missed work on the following day due to related back pain.

Pneumoconiosis - Premier Elkhorn Safety Dept. personnel were notified that the former employee had filed a claim.

Employee left foot became lodged in mud. When he tried to dislodge it by jerking his leg, he struck his left knee against a belt rail.

Employee was lifting a continuous miner cable while on his knees and strained his back while attempting to stand.

Employee dropped rock duster on his right hand, crushing & lacerating his middle finger.

EE was tramming shuttle car when the miner operator pulled his cable too tightly, pulling it down from hangers in front of the shuttle car. The shuttle car operator then went around his car to re-hang the cable when his right knee popped.

Employee was cleaning off roof bolt machine when he pulled on some loose wire that moved underneath his safety glasses causing lacerations to his right eye.

Employee was setting timbers when one fell over, striking the middle finger of his left hand resulting in multiple fractures.

An unintentional roof fall occurred in the #11 entry 30 feet outby spad #3690. The fall measured approximately 25'-30' long x 17' wide x 4'-6' high. The affected area was dangered off.

Employee was loading belt structure onto a 4-wheeler when he experienced pain in his left shoulder. The accident wasn't deemed reportable until the employee began taking off work due to complications from this injury on Novemember 10, 2008

On 6/24/2008, a piece of draw rock struck the employee's head, neck & back. Immediately following the accident, the employee was treated & released from a local hospital, but has since sought medical treatment. Then, he began taking off work on 5/18/2009 as a result of this accident/injury.

Employee was in the operator's deck backing the Fletcher Roof Bolter out of the #8 heading, when he got his Left ring finger caught between a trailing cable and the frame of the roof bolter, causing a laceration that required stitches.

Employee was loading oil cans onto the RamCar when he dropped one can & reached to retrieve it. The can had an upturned tab on the lid, lacerating the employee's left pinky finger and requiring stitches.

An unintentional roof fall was discovered at 5:30 am on Monday, September 17, 2007. The fall occurred at Spad # 2865, 2500 feet outby the working section. The fall measured approximately 20 feet wide x 70 feet long x 5 feet high. Additional support has been added to this area to prevent future roof falls.

The Storm Security Guard was sitting in a chair at the mine office when he suddenly fell into the floor. EMTs & METs at the site tried unsuccessfully to revive the victim until an ambulance arrived, which transported him to Jenkins Community Hospital.

EE was injured on 7/27/06. He returned to work on 8/9/06 on regular duty. He stated that his shoulder started hurting again. On 8/25/06.

Employee was shoveling belt line when his shovel caught the belt pulling his shoulder muscle. He went to dr. They put it in a sling.

Employee was raising the boom on mechanics buggy when he let the boom fall, smashing his left ring finger.

Employee was operating roof bolter, while changing drill steel, a piece fell out of hole striking his rt. little finger.

EE was pulling on a roof bolt to straighten it & strained his back.

EE WAS CHANGING BATTERIES AND FELT PAIN IN GROIN AREA. HE CONTINUED TO WORK UNTIL 1-29-04 AND HAS NOT YET RETURNED TO WORK AT THIS TIME.

WHILE REPAIRING MAT CHAIN ON MINER A PIECE OF ROCK FELL FROM THE ROOF STRIKING HIM ON BACK OF NECK AND HEAD CAUSING CONTUSIONS.

WHILE WORKING ON SPEED REDUCER IT DROPPED ON RIGHT INDEX FINGER CAUSING A FRACTURE.

WHILE OPERATING SHUTTLE CAR HIT HOLE IN ROADWAY HE STARTED COMPLAINING OF BACK PAIN

WHILE DRILLING A HOLE TO INSTALL ROOF BOLT A PIECE OF DRAW ROCK FELL STRIKING HIS LEFT SHOULDER CAUSING A CONTUSION.

WHILE OPERATING ROOF BOLTER A PIECE OF DRAW ROCK FELL ON HIS HAND CAUSING A FRACTURE TO ONE FINGER.

EE WAS DRILLING A HOLE AND WHEN HE REACHED FOR HIS LEVER TO LET THE HEAD DOWN HE GOT THE WRONG ONE AND HIT THE BOOM SWING LEVER AND IT SUNG OUT CAUSING HIM TO TWIST HIS BACK. THE LEVERS HAD BE EN MOVED RECENTLY PER K.D.M.M. AND MSHA.

EE WAS HOLDING A PIN WHILE ANOTHER EE USED A HAMMER TO INSTALL CHAIN AND THE HAMMER GLANCED OFF AND HIT EE MIDDLE FINGER ON HIS RIGHT HAND BREAKING IT. EE WORKED NEXT TWO DAYS AND DID NOT SEEK DOCTORS CARE BUT ON 11-13-02 HE WENT TO DOCTOR FOUND BREAK.

WHEN BOLTING TOP FELT PAIN IN LOWER BACK.

WHILE BOLTING, EMPLOYEE MOVED ROOF BOLTER HEAD OUT AND CAUGHT HIS FINGER ON HIS LEFT HAND BETWEEN MINE ROOF AND BOLTER CAUSING LACERATION.

WHILE INSTALLING CURTAIN BOARD HE HIT HIS HEAD. HIS MISSED NO TIME AND REQUIRED NO REPORTABLE TREATMENT. ON 10-15-02 HE WENT TO DR COMPLAINING ABOUT NECK PAIN.

WHILE PLASTERING A BRATTICE HE GOT PLASTER IN HIS EYE

HE WAS LOADING A BOX OF GLUE ONTO A ROOF BOLTER WHEN HE EXPERIENCED PAIN IN HIS LOWER BACK. X RAYS SHOVEL CLEAR DR TOLD HIM TO TAKE 3 DAYS OFF.

WHILE PULLING MINER CABLE HE STRAINED HIS BACK.

BENT OVER WALKING, PULLING MINER CABLE, TWISTED RIGHT KNEE. HE CONTINUED WORKING UNTIL 3/7/02 & HAS BEEN OFF WORK SINCE 3/7/02.

WHILE INSTALLING ROOF BOLT THE EE GOT HIS RT. HAND BETWEEN ROOF BOLTER AND ROOF BOLT CAUSING LACERATIONS.

WHILE SWINGING OVER TO PUT UP RIB BOLT AND TWISTED LOWER BACK.

WHILE BOLTING TOP A PIECE OF DRAW ROCK FELL ON THE EMPLOYEE'S BACK, BREAKING A RIB.

WHILE BOLTING TOP A PIECE OF DRAW ROCK FELL ON EMPLOYEE'S HAND BREAKING HIS FINGER.

ROOF BOLTER RAN OVER A ROCK PUSHING EE HEAD INTO MINE ROOF CAUSING SOFT TISSUE DAMAGE TO NECK.

EMPLOYEE WAS BOLTING MINE ROOF WHEN HE MOVED THE BOLTER HEAD TO INSTALL SECOND BOLT A PIECE OF DRAW ROCK FELL, STRIKING HIM ON THE HEAD AND BACK.

EMPLOYEE WAS MOVING A MANTRIP AND THE THROTTLE STUCK AND HIT THE MINE RIB CAUSING STRAIN TO HIS LOWER BACK AND NECK.

EE WAS BOLTING ROOF IN #1 ENTRY WHEN A PIECE OF DRAW ROCK FELL STRIKING HIM AND PUSHING HIM TO THE MINE FLOOR CAUSING A FRACTURE TO HIS NOSE AND LEFT ANKLE.

THE EE WAS OPERATING A 21SC SHUTTLE CAR. THE CAR BOUNCED CAUSING HIM TO HIT HIS ELBOW AGAINST THE CAR CAUSING A BRUISE.

EE WAS GETTING OFF A MANTRIP AND ACCIDENTALLY DEPRESSED THE ACCELERATOR. HE WAS THROWN BACKWARDS AND GOT HIS BACK AND SHOULDER PINNED AGAINST THE MANTRIP AND THE MINE ROOF CAUSING A CONTUSION.

VICTAM WAS BOLTING TOP WHEN A PIECE OF ROCK FELLSTRICKING THE VICTIM ON THE SHOULDER & SLLIDING DOWN THE LEFT ARM

EE WAS INSTALLING A ROOF BOLT AND A SMALL PIECE OF DRAW ROCK FELL AND MADE A SMALL CUT ON HAND.
