A scale clerk arrived at the office to find a goat on the property. When EE and other employees attempted to enter, it head-butted them. The plant manager attempted to lead the goat off the property. It turned & started back towards the office, EE grabbed one horn and the goat jerked its head, causing a puncture wound to the right hand resulting in an injury requiring 4 stitches.

The injured employee along with a co-worker were making repairs to the sand screw. Both employees were welding within close proximity to one another. Although the injured employee was wearing the correct PPE, he suffered welding flash burns to his eyes.

THE OPERATOR OF A VOLVO A35C HAUL TRUCK WAS NOT WEARING HIS SEAT BELT. AS A RESULT OF THAT, IT IS MY OPINION THAT HE HIT HIS HEAD ON A METAL POST INSIDE OF CAB, CAUSING HIM TO LOSE CONTROL OF THE TRUCK.

HE WAS TRE ATED AND RETURNED TO WORK. ON MAY 11, HE WAS TOLD SURGERY WOULD BE NEEDED. SURGERY WAS PERFORMED ON 6/5/92. HE WORKED REGULAR JOB UNTIL 6/5/92.
