Miner was pressing a pin into a dozer track. The press malfunctioned and the saddle tool of the press hit the miners right forearm resulting in a fracture of the Ulna.

The miner was going to utilize a rare earth power lift magnet weighing approximately 65lb. They placed the strap of the magnet over right shoulder to carry it to a piece of steel, they then turned to walk and struck left foot on a sawhorse losing balance and falling onto the concrete floor with the left side of their body, the magnet landed on their right hip.

Miner was ripping the pit bench on a Dozer with the blade in the raised position, when he came to end of the bench he continued off the steep slope and the abrubt force threw the Miner into the friction stearing levers making contact with his head/face causing minor contusions and bruises.

The Miner had a 1/2" cotton rope coiled around his left forearm, as he drove the JCB forklift forward, the tail end of the rope went under the left rear tire, pulling the miners left arm down against the door jamb breaking the bones in his left forearm.

State Scale Inspector pulled his truck next to scale stepped out on wall. Began to walk toward another truck and lost footing and slipped and fell between truck and wall. Possibly hitting head or body on truck bumper.

Operator was pushing material with dozer parallel with edge of slope. Material gave way on right side of dozer track. Dozer rolled down a 30 foot slope, turning over coming to rest on its track.

After completing a repair on the deep pit belt, EE was trying to release the clamps holding the belt in place. He got on the belt & released the clamps and the slack which was in the belt moved the belt forward & took him upside a roller which pinned him at the chest for about 1-3 min until a co-worker cut the belt and removed him from roller.

INSTALLING RETURN WATER LINE FROM DERRICK SCREEN TO PONDS. EE WAS HELPING INSTALL THE PIPE, HOLDING ONTO THE PIPE TO THE PLACEMENT BEAM WHEN IT MOVED AND PINCHED HIS MIDDLE FINGER IN BETWEEN THE PIPE AND THE BEAM. CUTTING TIP OF THE FINGER ON HIS LEFT HAND.
