EMPLOYEE PROCEEDED TO EXCHANGE THE CUTTING EDGE ON THE LOADER. HE CUT THE MIDDLE OUT FIRST WITH A TORCH AND LEFT THE TWO ENDS. AFTER CUTTING THE ENDS LOOSE WHICH SHOULD HAVE MADE THE CUTTING EDGE DROP, BUT IT DID NOT. THE EMPLOYEE TOOK A METAL BAR AND TRIED TO PRIZE IT ON OFF THE EDGE BROKE LOOSE AND FELL.
