MARTIN BODLEY WAS STANDING ON A STEP LADDER HELPING TO LOWER A MOTOR AND GEAR BOX THAT HAD BEEN REMOVED FROM A SHAFT IN YELLOWCAKE. THE CHAIN ON THE COME-A-LONG WAS CATCHING AND MARTIN TAPPED IT WITH A HAMMER TO LOOSEN IT. THE CHAIN CAME LOOSE & FREE-WHEELED WHICH ALLOWED MOTOR/GEAR BOX TO BOUNCE OFF THE SAFETY RAIL & HIT THE LADDER MARTIN WAS STANDING UPON. MARTIN JUMPED FORWARD
