EE was engaged in the task of removing a slurry pipe at the Boar Hammock Mine. EE attempted to cross over the pipe to reach the other side. Instead of stepping over the pipe, EE stepped onto the pipe to cross. The pipe had a slippery residue on its surface, causing EE's foot to slip. EE heard and felt a "pop" in EE's left knee.

On 07/29/2025 around 8:00 AM employee was working on the dredge loosening a hydraulic proportional control valve. Employee was pulling the wrench with left arm and heard a popping sound. Left forearm looks visually swollen. And they said that they have finger movement but cannot move the arm without severe pain.

EE was coming down from the back of the water truck with a hose in hand. The high velocity fire hose valve opened up and the employee lost their footing. EE did make it to the ground on both feet, but the landing was hard.

Employee was placing their hard hat on their head. They believed they got bit by ants, however, the swelling got worse and it appears to be more than ant bites. Employee sought medical care where a prescription was provided to treat the skin condition.

EE was fueling a mini excavator (yanmar v080 mini) and lost footing on the track and hit head on the fuel truck (international htx) bumper that was being used to fuel the excavator.
