The drill crew was tripping out HWT casing onto rod racks. As the casing was sliding down the slide it became wedged between 2 lower 20' sections of casing. The injured worker had to lift it out of the wedge and in the process they hurt their knee while straining.

Mechanic was unbolting a 992G loader bucket lip protector. When loosening the nut the bolt begin to spin making contact with left leg causing a laceration just above the knee requiring three staples. Employee was properly treated and released to return to work that same day.
Lifting
Staff member donned partial refinery PPE, leaving off the chaps/leg protection. Some hot slag splashed, entered EE's boot, causing 3rd degree burns to EE's right foot. Medical treatment was sought with follow up pending.
Lifting drilling supplies & strained shoulder
While sweeping was being done in the Administration Building, a staff member suffered an asthma attack believed to have been triggered by dust particles in the environment. Staff member was transported and treated by medical personnel in Hawthorne.

Employee walking from the conveyor to the stacker stepped on uneven ground and rolled ankle resulting in a strain.
Employee escorted a delivery driver with steel flat bar, standing too close in proximity, the steel bounced landing on right foot, fracturing three bones.
Miner slipped from the 2nd rung of 3 foot ladder when EE lost grip while tightening a bolt on a pump. EE fell to the concrete floor and suffered bruising to lower back and hip. Ladder was dry but 3 points of contact had not been maintained.

Employee was descending a slope into a dry pond when EE felt a pop in left leg, suffered a ground level fall, and noticed leg would not support EE's weight when EE tried to get up. EE was taken in for evaluation and returned to work for the next normal shift by the Physician. Worked until surgery on 10/17/2019.

Miner was transferring nitric acid from tote to barrel. He did not have his respirator, face shield or tyvek suit on. Splashed onto his leg and his face. Also felt he got fumes in his lungs and became light headed. Immediately decontaminated and taken to hospital for evaluation. Return to work same shift.
Miner was disconnecting conveyor section. Placed body in wrong position. Connectors came loose and pinched miners left hand. Return to work same day.
Miner looked under tail assembly and some material came dislodged. A small quantity of cement contacted his left eye. The eye was flushed by medical responders on site and cleared of any debris and then he was taken to ER for evaluation. No damage to the eye was found and he will be released to return to full duty on 1-31/2014.

Delivery driver for Transwood Company fell off his truck and landed on a hose causing injury to his back.
Employee finished welding pipe together on a portable pipe welding machine and gave the ok to have his coworker pull the pipe through. Employee was standing next to the pipe welder when the pipe was being pulled through. The tire was pulled over employees left ankle.
Employee working at crusher was walking along the belt line when he tripped over a piece of iron that was laying on the ground. The employee fell into the frame of the conveyor striking his face on the side of the frame.

An employee cut his left thumb while trying to cut drip tubing. the employee was utilizing a utility knife.
An employee was removing material from the top of a tail pulley conveyor guard, the employee had his left knee rested on the tail pulley guard and his right foot on the tow bar of that conveyor. The employees right foot slipped and contacted the ground causing the employee to twist his right knee. The employee's right foot was approximately 1' of the ground.
An employee was removing excess slag after a weld utilizing a wire wheel attached to a grinder. A piece of slag ricocheted and landed in the employees eye causing trauma to the cataract. The employee was wearing safety glasses and a face shield.

An employee got his thumb pinched by the forks of a forklift while spotting the forklift during a conyeyor move.
An employee was attempting to remove a dirt clump from a stacker conveyor, the employee was positioned at the tail end of the conveyor, standing above the dead pan. The employee lost his footing, fell onto the moving conveyor and was deposited of the end of the conveyor. The employee dropped approximately 4', resulting into a compression fracture to the L1 &L4 vertebrae.

AS THE NUT BACKED OFF THE BOLT, THE BOLT DROPPED OUT OF THE RECESS, ALLOWING THE BOLT TO ROTATE INTO THE PALM OF HAND BY THUMB.

HE WAS WALKING ON LINER WITH WELDER IN HIS HAND SLIPPED & FELL WITH WELDER LANDING ON FINGER.
HE PULLE UP TO RESTROOMS ADN GOT OUT OF TRUCK AND GRABBED ON TO HAND RAILS TO PROCEED DOWN STAIRS WHEN HE LOST HS GRIP AND FELL TO GROUND HURTING HIS LEFT LEG.
DRILL HELPER BACKED THE WATER TRUCK UP TO THE DRILLING RIG AS HE WENT TO GET OUT OF THE TRUCK HE LEFT THE TRUCK IN REVERSE AND LET UP ON THE CLUTCH THIS CAUSED THE TRUCK TO ROLL BACK PINCHING THE VICTIM AGAINST THE TURN TABLE OF THE DRILL
