Borealis Mine: 120 MSHA citations Metal/Non-Metal

The drill crew was tripping out HWT casing onto rod racks. As the casing was sliding down the slide it became wedged between 2 lower 20' sections of casing. The injured worker had to lift it out of the wedge and in the process they hurt their knee while straining.

Mechanic was unbolting a 992G loader bucket lip protector. When loosening the nut the bolt begin to spin making contact with left leg causing a laceration just above the knee requiring three staples. Employee was properly treated and released to return to work that same day.

Lifting

Staff member donned partial refinery PPE, leaving off the chaps/leg protection. Some hot slag splashed, entered EE's boot, causing 3rd degree burns to EE's right foot. Medical treatment was sought with follow up pending.

Lifting drilling supplies & strained shoulder

While sweeping was being done in the Administration Building, a staff member suffered an asthma attack believed to have been triggered by dust particles in the environment. Staff member was transported and treated by medical personnel in Hawthorne.

Employee walking from the conveyor to the stacker stepped on uneven ground and rolled ankle resulting in a strain.

Employee escorted a delivery driver with steel flat bar, standing too close in proximity, the steel bounced landing on right foot, fracturing three bones.

Miner slipped from the 2nd rung of 3 foot ladder when EE lost grip while tightening a bolt on a pump. EE fell to the concrete floor and suffered bruising to lower back and hip. Ladder was dry but 3 points of contact had not been maintained.

Employee was descending a slope into a dry pond when EE felt a pop in left leg, suffered a ground level fall, and noticed leg would not support EE's weight when EE tried to get up. EE was taken in for evaluation and returned to work for the next normal shift by the Physician. Worked until surgery on 10/17/2019.

Was lifting HDPE pipe and lifted and turned incorrectly. Strained back muscle.

Miner was transferring nitric acid from tote to barrel. He did not have his respirator, face shield or tyvek suit on. Splashed onto his leg and his face. Also felt he got fumes in his lungs and became light headed. Immediately decontaminated and taken to hospital for evaluation. Return to work same shift.

Miner was disconnecting conveyor section. Placed body in wrong position. Connectors came loose and pinched miners left hand. Return to work same day.

Miner looked under tail assembly and some material came dislodged. A small quantity of cement contacted his left eye. The eye was flushed by medical responders on site and cleared of any debris and then he was taken to ER for evaluation. No damage to the eye was found and he will be released to return to full duty on 1-31/2014.

Delivery driver for Transwood Company fell off his truck and landed on a hose causing injury to his back.

Employee finished welding pipe together on a portable pipe welding machine and gave the ok to have his coworker pull the pipe through. Employee was standing next to the pipe welder when the pipe was being pulled through. The tire was pulled over employees left ankle.

Employee working at crusher was walking along the belt line when he tripped over a piece of iron that was laying on the ground. The employee fell into the frame of the conveyor striking his face on the side of the frame.

An employee cut his left thumb while trying to cut drip tubing. the employee was utilizing a utility knife.

An employee was removing material from the top of a tail pulley conveyor guard, the employee had his left knee rested on the tail pulley guard and his right foot on the tow bar of that conveyor. The employees right foot slipped and contacted the ground causing the employee to twist his right knee. The employee's right foot was approximately 1' of the ground.

An employee was removing excess slag after a weld utilizing a wire wheel attached to a grinder. A piece of slag ricocheted and landed in the employees eye causing trauma to the cataract. The employee was wearing safety glasses and a face shield.

An employee got his thumb pinched by the forks of a forklift while spotting the forklift during a conyeyor move.

An employee was attempting to remove a dirt clump from a stacker conveyor, the employee was positioned at the tail end of the conveyor, standing above the dead pan. The employee lost his footing, fell onto the moving conveyor and was deposited of the end of the conveyor. The employee dropped approximately 4', resulting into a compression fracture to the L1 &L4 vertebrae.

EMPLOYEE LIFTED A 4" YELLOW MINE PIPE WITH HIS BACK NOT HIS LEGS. SOME PULLED MUSCLES WERE HIS INJURY.

GEMINI TRUCK BEING LOADED WITH ROLLS OF PAD LINER WEIGHING APPROX 2800# EACH. I WAS COUNTING TO SEE HOW MANY ROLLS THERE WERE TO BE LOADED. AS I REACHED FIFTH ROLL THAT WAS BEING LOADED BY FOR K LIFT OPERATED BY ANOTHER EMPL I SAW INJ STANDING IN FRONT OF ROLL PULLING IT OFF OF FORKLIFT. ROLL FELL APPROX 2' ONTO BED OF TRUCK & BOUNCED UP ONTO BOARDS THAT WERE ON TRUCK TO PREVENT IT

LOADING JONES PLITTER ONTO PIPE TRUCK WITH EMPL EMPL. HE LOST GRIP ;& SPLITTER FELL OFF OF TRUCK PULLING EMPL'S RIGHT ARM BEHIND HIM.

WHILE DRILLER OF LDC4027 DRILL I NAME WAS TRYING TO BRAKE OFF HAMMER BIT & CHANGE OVER TO A TRICONE BIT HE PLACED HIS 60 INCH PIPE WRENCH ON HAMMER TO BREAK IT OFF & WHEN HE STARTED HIS ROTATI ON 60 INCH PIPE WRENCH SLIPPED OFF THE BRAKE OUT& ROTATED TOWARDS HIM WHILE HE WAS SITTING IN THE CAB WRENCH EXTENDED INTO THE CAB & STRUCK IN THE LOWER PART OF THE LEG JUST ABOVE THE BOOT

THE INJURED WAS PUSHING ORE OFF A STOCKPILE WITH A CABLESS DOZER AND APIECE OF DIRT GOT INTO HIS RIGHT EYE.HE WAS WEARING SAFETY GLASSES AT THE TIME OF THE ACCIDENT.

ANOTHER MECHNIC WAS REMOVING NUTS FROM LOBTS OF THE LEFT LIP PROTECTOR PLATE ON THE BOTTOM OF BUCKET WITH A IMPACT WRENCH. HE WAS HOLDING THE BOLT IN THE BOLT HEAD RETAINING RECESS ON THE BAC K SIDE OF BUCKET. AS THE NUT BACKED OFF THE BOLT, THE BOLT DROPPED OUT OF THE RECESS, ALLOWING THE BOLT TO ROTATE INTO THE PALM OF HAND BY THUMB.

CREW WAS TRIPPING PIPE OFF OF PIPE TRUCK TO ROD BOOM EMPL SLIPPED IN THE MUD

CREW WAS BOOMING DOWN A LOAD OF DRILL PIPE. CHEATER PIPE ON THE CHAIN BOOMER SLIPPED AND EMPLOYEE FELL OFF THE BACK OF THE TRUCK.

THE INJURED AND THE DRILLER WERE TRYING TO REMOVE THE HAMMER FROM UNDERNEATH THE DECK OF THE DRILL.THE HAMMER WAS ATTACHED TO A CABLE SO THEY COULD PULL IT OUT AND UP AND SET INTO APOCKET.TH E HAMMER WAS BOUND UP AND AS HE REACHED TO PULL IT OUT,THE HAMMER POPPED OUT AND HIT HIM IN THE MOUTH.

EMPLOYEE ATTEMPTED TO PICK UP A HEAVY BOX FROM VAN & PLACE IT IN A PICKUP TRUCK SHE TURNED WITH LOAD AS IT SLIPPED DOWN ONTO BED & SHE SUFFERED A LOWER BACK STRAIN SHE DIDNT FELL ANY PAIN U NTIL THE FOLLOWING MORNING THEN SHE REPORTED THE INCIDENT SHE WAS TAKEN TO THER DOC.EXAMINED & PLACED ON LIGHT DUTY FOR 2 DAYS

I. NAME WAS PICKING UP HIS IMPACT WRENCH WITH AIR HOSE ATTACHED TO IT. HE HAD HISI SAFETY GLASSES ON AT THE TIME, BUT IT WAS IN A SHADED AREA, SO HE REMOVED THEM SO HE COULD SEE. WHEN PICKIN G UP THE IMPACT WRENCH, THE QUIK DISCONNECT POPPED OFF, HITTING HIM IN THE EYE.

HE WAS WALKING ON LINER WITH WELDER IN HIS HAND SLIPPED & FELL WITH WELDER LANDING ON FINGER.

HE PULLE UP TO RESTROOMS ADN GOT OUT OF TRUCK AND GRABBED ON TO HAND RAILS TO PROCEED DOWN STAIRS WHEN HE LOST HS GRIP AND FELL TO GROUND HURTING HIS LEFT LEG.

DRILL HELPER BACKED THE WATER TRUCK UP TO THE DRILLING RIG AS HE WENT TO GET OUT OF THE TRUCK HE LEFT THE TRUCK IN REVERSE AND LET UP ON THE CLUTCH THIS CAUSED THE TRUCK TO ROLL BACK PINCHING THE VICTIM AGAINST THE TURN TABLE OF THE DRILL

EMPLOYEE WAS WA5KING DOWN A SET OF STEPS WITH SNOW ON BOTTOM OF HIS BOOTS HE SLIPED & ROLLED DOWN STEPS LANDING ON HIS LEFT WRIST HE SUSTAINED A SPRAIN OF THE WRIST

3 EMPLOYEES WER9 INFLATING A 16 INCH TIRE ON A U6 1/2 INCH RIM. THE BEAD WOULD NT POP OVER THE SHOULDER OF THE RIM. AFTER INFLATING IT TO AN ESTIMATED .0 PSI I NAME LEANED OVER THE TIRE TO REL EASE THE AIRCHU7K. THE TIRE THEN EXPLODED, THROWING 22 OFF HISFEET AND BREAKING I NAME ARM AND CUTTING HIS NOSE.

EMPLOYEE WAS CLIMBING INTO THE 988B LOADER BY WAY OF THE LADDER PROVIDED BY THE MANUFACTURE. AS HE REACHED THE TOP OF THE LADDER, HE FELT A PAIN IN HIS LOWER BACK.

EE WAS GIVEN THE JOB OF REFUELING A PORT LIGHT PLANT. AFTER REFUELING HE STARTED TO DISMOUNT THE UNIT & SLIPPED LANDING ON THE GORUND APPROX 4\ & TWISTED HIS ANKLE.

IN CUTTING TRACK FROM DOZER, TRACK PARTED WITH A SIDEWAYS MOTION GRAZING EMPLOYEES LEG. X-RAYED CONFIRMS BRUISE. NO LOST TIME.

CRUSHER WAS RUNNING BUT WAS SOON GOING TO BE SHUT DOWN FOR MAINTENANCE. THE EE STARTED CLEANING IDLER WHICH HE WAS GOING TO REPLACE -BELT WAS STILL RUNNING. THE EE GOT HIS LEFT HAND CAUGHT THE IDLER & THE B ELT.
