EMPLOYEE WAS INSTALLING PIPE IN A WATER PUMP LOCATED ON THE BED OF THE MAINTENANCE TRUCK WITH A PIPE WRENCH. THE PIPE WRENCH SLIPPED OFF AND THE RIGHT HAND HIT THE TAILGATE OF THE TRUCK WHIC H RESULTED IN A FRACTURED RIGHT WRIST.

EE WAS GOING TO USE A TORCH TO REMOVE A PIECE OF STEEL THAT WAS NOT ALLOWING THE BIN GATE AT THE SECONDARY PLANT TO CLOSE PROPERLY. AS HE BEGAN THE DOOR STARTED CLOSING KNOCK THE TORCH BEING H ELD BY HIS RIGHT HAND LOOSE, MASHING HIS RIGHT HAND AND CUTTING HIS MIDDLE FINGER OF LEFT HAND.
