THE INJURED EE WAS ASSISTING ANOTHER EE REMOVE TRACKS FROM A DOZER AND WHILE HEATING BOLT FOR REMOVAL, GREASE POPPED OUT AND SPECKLED HIS FACE. EE WAS WEARING HIS SAFETY GLASSES AT THE TIME OF INCIDENT.
THE INJURED EMPLOYEE WAS ASSISTING WITH THE PLACEMENT OF PORTABLE POWER POLES WHEN THE RIP-RAP HE WAS STANDING ON SLIPPED OUT FROM UNDER HIS FEET CAUSING HIM TO FALL TO THE GROUND.

WHILE INSTALLING CRUSHER,, THE CATWALK SEPARATEDFROM CRUSHER FRAME AND EE AND CATWALK FELL APPROX 3 FT TO GROUND LEVEL.
THE DRILLER WAS REMOVING THE CENTRALIZING BLOCKS FROM THE DRILL STEEL, WHEN THE MAIN CENTRALIZING COLLAR SLIPPED DOWN THE DRILL STEEL SMASHING HIS FINGER BETWEEN THE COLLAR AND BLOCK.
