The employee exited the fuel truck and slipped and hit his shoulder against the fuel tank on the truck. The employee was seen by a doctor on 11/16 for an MRI and was put onto modified duty on 11/17/04. The employee has a contusion of his left shoulder.
Employee was exiting the track hoe and improperly stepped down twisting his left ankle. Resulting in an ankle sprain.

EE WAS ATTEMPTING TO CHECK HIS LOADER, WHILE DOING SO THE EE SLIPPED AND FELL. THIS FALL CAUSED THE EE TO INJURED HIS WRIST AND BACK.

EMPLOYEE WAS CLIMBING DOWN FROM HIS HAUL TRUCK WHEN HE SLIPPED CAUSING HIM TO START TO FALL. THE EMPLOYEE REACHED UP ATTEMPTING TO CATCH HIMSELF WITH HIS LEFT HAND, THIS CAUSED THE EMPLOYEE TO STRAIN HIS LEFT WRIST. EMPLOYEE WAS NOT MAINTAINING A THREE POINT OF CONTACT.
EMPLOYEE WAS ATTEMPTING TO REPAIR AN AIR COMPRESSOR. WHILE DOING SO HIS WRENCH SLIPPED CAUSING HIM TO STRIKE HIS KNUCKLES ON A PIECE OF METAL. HE CUT HIS HAND & REQUIRED 4 STITCHES.
EMPLOYEE WAS WORKING ON A SHAKER SCREEN WHEN THE WIND BLEW AN OBJECT INTO HIS EYE. THE EMPLOYEE WAS WEARING SAFETY GLASSES AT THE TIME OF THE ACCIDENT.
