HAVING TROUBLE WITH COAL SYSTEM, WAS CHECKING OUT ROTARY VALVE, AND PUT RIGHT HAND IN VALVE WHILE IT WAS RUNNING, OPERATOR PULLED DISCONNECT SWITCH AFTER ACCIDENT.
