While adjusting the cone and wiping the number face of shim #4, the shim had a sharp sliver which went through his glove and cut his right index finger. At the time this employee did not feel he needed medical attention. On 10/05/05 he requested medical attention for this injury.

A ROCK APPROXIMATELY 12' FELL OUT OF THE SCREEN AND HIT THE TOP OF HIS LEFT FOOT.
HE WAS ASSISTING IN A LINER CHANGE AND WHILE REMOVING BOLTS HIS CO-WORKER WAS USING AN IMPACT GUN TO REMOVE THE BOLTS, SUDDENLY A NUT FLEW TOWARD HIM STRIKING HIN IN THE MOUTH.

WHILE EE WAS PULLING REJECT ROCK FROM THE FEEDER. A ROCK FELL FROM THE BUCKET AND HIT THE BUCKET ARM AND WINDSHIELD, WHICH SENT SMALL PIECES OF GLASS INTO HIS FACE.
HE WAS SWINGING A HAMMER TO PUNCH HOLES IN BELTING. THE HAMMER HEAD CAME OFF AND HIT HIM ON HIS LEFT HAND FINGER.

HE CRAWLED UNDERNEATH THE WATER PULL TO REPAIR THE FENDER. WHEN HE CAME OUT FORM UNDER THE ACHINE HE FELT A BAD PULL AND SHARP PAIN ON HIS R-GROIN AREA.
HE INJURED HIS KNEE WHILE TURNING TO THE LEFT AND WHILE GETTING OUT OF THE LUBE TRUCK.
HE WAS WELDING A PLATE ON CRUSHER #6 FEEDER. EMPLOYEE WAS GRINDING ON THE OTHER SIDE OF THE PLATE. EMPLOYEE WAS GETTING THE FLASH FROM THE WELDER.
