EE STATES HIS FOOT SLIPPED AS HE WAS TIGHTENING THE BOLTS ON THE CRUSHER JAW WING, CAUSING HIS BACK TO TWIST.
EMPLOYEE STATES HE WAS HELPING PUT A HOPPER TOGETHER AND WAS STANDING ON A PLATE WAITING FOR ANOTHER EMPLOYEE TO PUT BOLTS IN THE CONVEYOR BELT. THE BELT SHIFTED AND HIS LEGS WERE PINNED AGAIN ST THE FEEDER BIN GATE CAUSING CONTUSIONS AND ABRASIONS TO BOTH THIGHS.
