THREE MAN CREW WAS ATTEMTPING TO CLEAR A CONE CRUSHER PLUG-UP. THE HEAD BOX HAS TO BE RIASED TO ALLOW ACCESS TO CLEAR THE CRUSHER. THE HEAD BOX WAS LIFTED BY A BOOM (HYDRAULIC) TRUCK AND BRACI NG WAS PLACED UNDE THE HEAD BOX. WHEN THE HEAD BOX WAS LOWERED THE BRACING SLIPPED AND THE INDIVIDUALS LEFT RING FINGER WAS PINCHED BETWEEN THE HEAD BOX AND THE I-BEAM.
