While employee was moving 55 gallon drum of Vinyl. Employee pinched a finger between 2 drums.

There was spilled Green Sand on pallet. Employee wiped the green sand off of the pallet with hand.

Employee stacking bagged material on a pallet and palletizing the pallet. Employee states low back pain.

Employee was using a wrench to loosen a bolt. While wrenching the bolt ee lost balance and fell causing a knee strain.

Employee was filling a sand bag when a bad bag broke and hot sand hit arm causing a 1st aide irritation.

Employee was replacing tube to a nozzle for our sand blending system and had cut arm on the metal bracket while replacing the tube.

Employee was using hand tools, a 12" bar and a hammer to dislodge a piece of angle iron from a structure. As the angle iron was dislodged EE's finger was pinched between the bar and the ground.

Employee was leaning over to retrieve a rope to get levels from the silos, causing mid back pain

Employee was climbing down the silo ladder, his footing slipped and he scraped his back on the ladder cage.

Employee did not keep a safe distance from a welding torch

Employee was stacking 50# bags of our product #16 Blastoff (sand) onto a pallet. While adjusting the under layer of bags he felt a pop in his left wrist.

Repetitive movement. Employee was stacking 100 lb sacks of Pool Sand when he felt pain in his right wrist and elbow. Employee was diagnosed with tennis elbow and ganglion cyst wrist.

While working with a socket wrench, his hand slipped, causing a sprained wrist.
