Employee was performing housekeeping, when they picked up a cardboard box with parts from the floor. The box weighed approx. 35 lbs., employee complaint of lower back pain. Employee was taken to the ER for an evaluation and was diagnosed with a lower back strain.

EMPLOYEE WENT UP A STATIONARY BELT CONVEYOR TO GREASE THE HEAD PULLEY. THE BELT STARTED AND THREW HIM OFF OVER THE CHUTE AND ONTO A SPIRAL SAOLT CLASSIFIER, THAT WAS RUNNING SUFFERED A SEVEREL Y LACERATED DELTOID MUSCLE OF HIS RIGHT ARM, BRUISES AND ABRASIONS ON HIS CHEST AND ARMS AND LEGS.
