Miner lost mental focus and smashed his thumb with a spike (track) maul while spiking rail. Injury consisted of a fractured and contused thumb for which the miner was prescribed pain medication.
Drill helper was holding drill rod and casing sub below rod holder so driller could reposition drill head. Helper failed to release the rod/sub as quill rod was raised. Helper's hands were drawn into the rod holder causing lacerations/avulsions to both hands and broken index finger on left hand.
Miner was training underground when co-worker's boots flung mud into miner's eye. Miner was not wearing proper eye-protection. Miner incurred no injury or damage to eye however he did receive prescription medication for pain and antibiotic.
Miner was barring down at drift face. A rock was brought down that bounced/rolled over employees foot. Employee was not wearing full metatarsal protective boots, just steel toed rubber boots. The impact of the rock created a laceration and broke two bones in the employee's foot.

Miner was undergoing task training as an assistant to the diamond driller at the operation. While performing supervised operation during production, Miner's right hand was crushed between the drill rod and a drill casing adapter (sub), resulting in lacerations and broken bone(s).

Miner was using a scaling bar to work an oversized rock through a chute. The rock moved in such a way as to pinch the miner's hand between the scaling bar and the back of the drift, resulting in a laceration. The miner received 5 sutures to close the laceration on the back of his hand. The miner returned to work the same day with no restrictions.
While checking/replacing the mine phone in the mine office, two dogs began to fight in the office. Victim and co-worker attempted to break up the dogs and remove them from the premises. As victim kicked at dogs, the co-worker swung a mine phone at the dogs, connecting with victim's leg. A minor laceration to victim's leg, requiring four (4) sutures resulted.
Repetitive motion injury/illness. No specific accident or incident. Miner has received initial diagnosis of "lateral epicondylitis" or tendonitis (commonly known as "tennis elbow").
