Castillo Ready Mix Inc · Caught in, under or between running or meshing objects

At -12:45 PM, EE cleaned a return idler while a conveyor was running, without supervisor approval. Despite a coworker's warning, EE continued and was pulled into the belt. Coworkers stopped the conveyor and freed EE. EE suffered minor abrasions. Cause: working on moving equipment without lockout/tagout.
