American Select Corp · Caught in, under or between running or meshing objects

Miner, plant operator, was attempting to free VSI crusher of debris from the grizzly. EE did not follow shut down procedure to perform said action; left conveyors running, which resulted in EE's jacket becoming entangled in head wheel shaft of the conveyor, causing EE to fall, resulting in a broken arm, which required surgery.
