EE WAS TIGHTENING BOLT ON SCREEN CLOTH WHEN THE WRENCH SLIPPED OFF. LOST HIS BALANCE, FELL BACK ONTO CONVEYOR APPROX 3 TO 4 FT, HIT CONVEYOR THEN FELL 4 TO 5 ' TO GROUND, CAUSING MULTIPLE BRUI SES AND POSSIBLY A CLOSED HEAD INJURY. DR SAID EE COMPLAINS OF HEADACHE.
