AN EMPLOYEE RECEIVED INJURIES TO HIS LEFT WRIST AND FOREHEAD AFTER HE COMPLETED CHANGING ELECTRICAL CABLE TO A CONVEYOR MOTOR. AFTER CONNECTING WIRING TO MOTOR, HE LOST HIS FOOTING DURING THE PROCESS OF REMOVING HIS TIE-OFF POINT. HE FELL FACE DOWN AND SLID APPROXIMATELY FOUR FEET.
EE WAS LIFTING A WOODEN PALLET INTO THE BACK OF A PICKUP FOR DISPOSAL. EE FELT A PAIN IN HIS BACK WHEN HE WENT TO PLACE PALLET IN TRUCK.

VICTIM BRUSHED AGAINST AN OVERHANGING TREE LIMB WHICH SPRANG BACK & STRUCK HIM IN THE RIGHT EYE.

EMPLOYEE WAS AT7EMPTING TO FREE A ROCK JAM GOING INTO THE PRIMARY CRUSHER FEEDER HOPPER WHEN A ROCK UNEXPECTANTLY MOVED STRIKING HIS RIGHT FOOT BEHIND THE STEEL TOE OF HIS SAFETY SHOES.
