AN EMPLOYEE RECEIVED INJURIES TO HIS LEFT WRIST AND FOREHEAD AFTER HE COMPLETED CHANGING ELECTRICAL CABLE TO A CONVEYOR MOTOR. AFTER CONNECTING WIRING TO MOTOR, HE LOST HIS FOOTING DURING THE PROCESS OF REMOVING HIS TIE-OFF POINT. HE FELL FACE DOWN AND SLID APPROXIMATELY FOUR FEET.

HE HAD REMOVED HIS DARK LENS SAFETY GLASSES DUE TO OVERACAST CONDITIONS & RAIN, BUT HAD NOT REPLACED THEM WITH THE CLEAR GLASSES HE WAS CARRYING IN HIS LUNCH BOX. VICTIM BRUSHED AGAINST AN OVERHANGING TREE LIMB WHICH SPRANG BACK & STRUCK HIM IN THE RIGHT EYE.
