Machine operator was operating a CAT D9T dozer clearing overburden material from a bench. Operator reversed the machine backwards an estimated 200-300 ft. at a high speed without looking back and without responding to honking from another machine, per witness testimony. Machine fell off an edge. Operator was pronounced dead at the scene. Machine is total loss.

Employee reports as they were descending the water truck their right foot was not placed properly on the equipment's step causing their right foot to roll as they stepped to the ground. Employee was diagnosed with a closed nondisplaced fracture of right foot.
Employee was climbing out of a chute with a lit torch in left hand when they swiped their right hand on the torch. Employee sustained a third degree burn to the right hand.

EE was exiting the crane cab, up and onto the crane deck when the EE caught their foot on an operational pedal; in the event the EE grabbed a cab door handle thus swinging the EE into the crane deck. The EE injured the right shoulder and knee in the while exiting up and onto the deck.

During the installation of rebar for the surge tunnel, a piece of #10 rebar was being placed and dropped. The bar was being installed by several men on a wall. The bar that fell was about 51'-0 long. It was being tied horizontally. When they were handing the bar across it slipped out and fell, landing on EE's wrist.

On 11/18/19 employee was diagnosed with a left chest wall contusion, left hip contusion, and acute costochondritis, and placed on restricted duty. On 10/3/19 employee was jarred inside the manlift basket resulting in pain in the chest.

On 11-14-13 the employee requested medical treatment, was taken to the Company clinic and was placed on restricted duty which is being accommodated. The employee alleged that operating the excavator and possibly the bobcat caused pain in his lower back.

Employee requested medical treatment on July 9th and was placed on restricted duty. Employee stated he was turning water valve using a wrench. During the process, the wrench was stuck and when he applied additional pressure, the wrench slipped off the valve and alleged he fell backwards. Diagnosed with lumbosacral sprain, lower and upper back contusion(s), and back abrasion.

The employee requested and received a medical evaluation by a company physician on 8/31/11. The employee was placed on modified duty and was diagnosed with a L-shoulder sprain. The employee alleges that he experienced pain to his L-shoulder when he lifted sample buckets.
Employee went to a dentist on 7/25/11 which required treatment/procedure beyond first aid. An unattached braided cable and shackle struck employee's lower lip and caused a lip laceration and fractured tooth.
