EE was using a hammer drill to drill into a concrete wall when the bit caught a piece of steel. The bit got stuck causing the drill to jerk his hands catching his fingers in the cord. This caused a break/dislocation to his right ring finger.

WHILE CLEANING ROCKS & DEBRIS FROM TOP OF GRIZZLY, EMPLOYEE'S FOOT SLIPPED BETWEEN BENT PIPING. (GRIZZLY IS BUILT WITH 2" PIPING SPACED APPROX. 3" APART). HEAVY ROCKS HAD BENT PIPING ALLOWING ENOUGH SPACE FOR HIS FOOT TO SLIP THROUGH (HE WAS WEARING HARNESS & LANYARD). RIGHT KNEE CAUGHT BETWEEN PIPING, TWISTING KNEE & CAUSING HIP PAINS.

WAS WATCHING FORKLIFT PICKUP A BUNDLE OF STEEL. BUNDLE WAS NOT CENTERED, SO HE LEANED OVER IT TOSEE WHY IT WAS NOT CENTERED. THE FORKLIFT HAD CAUGHT ON I BEAM AND IT WAS SLIDING SIDEWAY AND FE LL OFF ONTO HIS HAND CUTTING OFF THE TIPE OF HS TOW MIDDLE FINGERS ON HIS LEFT HAND.
